• I have consistently lived and/or worked in Florence, Reedsport or Mapleton for the past 12 months. 
• This vehicle is my primary source of transportation. 
• I do not have the financial means to pay for the needed repair(s).
• I have not received assistance from Siuslaw Transportation Solutions (formerly Rods' N Rhodies) within the past 24 months. 
• The vehicle in need of repairs is registered to me and has a current Oregon registration.
